Malawian men shun circumcision in Kasungu

The majority of men in Kasungu district are reportedly shunning circumcision which is being offered by health officials in the area, Malawi24 has been tipped.

Medical district health officer for Kasungu, Joremy Nkhambure said since 2012 they have managed to circumcise 3000 men only which is way below their target of 21,000 circumcisions for the 3 year period.

According to Nkhambure, cultural beliefs are one of the sources contributing to lower number of circumcised people as it is the home of Chewas.

He has however attributed that although the medical officers have tried to civic educate the Kasungu residents, there have been no fruits coming out of it.

The circumcision statistics in the area has shown that only 500 men have been circumcised during 2015. Research by health experts shows that circumcision reduces the risk of contracting HIV/AIDS disease.

National Aids Commission (NAC) has since set a target of 28,000 circumcisions annually which the district health authorities are not sure of acheiving.

So far, evidence that Malawi24 gathered shows that men who undergo circumcision are most likely to be infected with HIV than the uncircumcised.

  5. Is it a must. Why should it be a very big concern? There are a lot of “real” issues of concern than this circumcision. It is as if there is something someone somewhere benefits from this practice apart from the benefit to the circumcised…

    1. My learned Doctor, the way the circumcision issue was introduced and the way it has been and it is being marketed, leaves people asking a lot of questions. No wonder there is low response and low uptake. There were mixed reactions when it was introduced and stories kept changing and they keep changing. Recently there have been stories about infection rate increasing at the time when we as a nation adopted the condom strategy and now this new one. Our researchers should do a little more homework; and let’s avoid wholesale adoption of strategies which were researched elsewhere. When it was introduced, I already had knowledge of it from Muhimbili College of Health Sciences. I knew the confusion that was coming. I would suggest that your marketing focuses on younger people and not older people. My dear Doctor, don’t set your targets too high for a product too difficult to market. Consider the cost-benefit analysis. Back to my earlier argument, it appears there is something pulling strings like the cases of same sex marriage and the so-called right for women to choose whether or not to a keep a pregnancy.   10. Many professionals have criticized the studies claiming that circumcision reduces HIV transmission. The investigators did not seek to determine the source of the HIV infections during their studies. They assumed all infections were heterosexually transmitted.

    Many HIV infections in Africa are transmitted by contaminated injections and surgical procedures. The absolute rate of HIV transmission reduction is only 1.3%, not the claimed 60%. Even if the claim were true, based on the studies, about 60 men had to be circumcised to prevent one HIV infection.

    Authorities that cite the studies have other agendas including political and financial. Research shows that circumcision causes physical, sexual, and psychological harm, reducing the sexual pleasure of both partners. This harm is ignored by circumcision advocates. Other methods to prevent HIV transmission (e.g., condoms and sterilizing medical instruments) are much more effective, much cheaper, and much less invasive. Even HIV/circumcision studies advise using condoms. With condoms circumcision adds no benefit to HIV prevention.

  12. Male circumcision is a dangerous distraction in the fight against HIV/AIDS.

    From a USAID report:
    “There appears no clear pattern of association between male circumcision and HIV prevalence—in 8 of 18 countries with data, HIV prevalence is lower among circumcised men, while in the remaining 10 countries it is higher.”
    (this will include men who were circumcised tribally rather than medically, but they and their partners may also believe themselves to be protected, and the whole rationale for the RCT’s into female-to-male transmission was a purported correlation between high rates of male circumcision and low rates of HIV)

    It seems highly unrealistic to expect that there will be no risk compensation. The South African National Communication Survey on HIV/AIDS, 2009 found that 15% of adults across age groups “believe that circumcised men do not need to use condoms”. This figure seems to have been unchanged in 2012.

    It is unclear if circumcised men are more likely to infect women. The only ever randomized controlled trial into male-to-female transmission showed a 54% higher rate in the group where the men had been circumcised:

    ABC (Abstinence, Being faithful, and especially Condoms) is the way forward. Promoting genital surgery seems likely to cost African lives rather than save them.

    Europeans don’t circumcise, South Americans don’t circumcise, Australians and New Zealanders used to circumcise but stopped, and less than half of North Americans circumcise. Why should Africans circumcise?

    “A new study of 314 female s-x workers (FSWs) in Makindye division found that more than half of respondents falsely believe that once a man is circumcised, protection is not necessary during s-x.”

    and from Malawi:
    “According to the report, HIV rates have doubled in Malawi moving from 10% to 20% in 1 year. Strangely, this has been the same period that Malawians have been manipulatively forced to go through circumcision in masses with the promise that it reduces the contraction of HIV.”
